DMCA Takedown Policy
Anesthify hosts user-generated content (forum posts, facility reviews, salary submissions). If you believe content on this site infringes your copyright, follow this procedure to file a DMCA takedown notice. Designated agent contact is below.
Filing a takedown notice
Send a written notice to our designated DMCA agent that contains, at minimum:
- A physical or electronic signature of the copyright owner or authorized agent.
- Identification of the copyrighted work claimed to be infringed.
- Identification of the material claimed to be infringing, with information sufficient to locate it (typically the URL of the page or post).
- Your contact information (name, address, phone, email).
- A statement that you have a good-faith belief the use is not authorized by the copyright owner, its agent, or the law.
- A statement under penalty of perjury that the information in the notice is accurate and that you are the owner or authorized to act on the owner's behalf.

Counter-notice
If your content was removed and you believe it was a mistake or misidentification, you may submit a counter-notice with the same level of detail above. We'll forward it to the original complainant; if they don't file suit within 10–14 business days the content may be restored.
Repeat infringers will have their accounts terminated under our standard Terms of Service.
