DMCA Takedown Policy

Anesthify hosts user-generated content (forum posts, facility reviews, salary submissions). If you believe content on this site infringes your copyright, follow this procedure to file a DMCA takedown notice. Designated agent contact is below.

Filing a takedown notice

Send a written notice to our designated DMCA agent that contains, at minimum:

  1. A physical or electronic signature of the copyright owner or authorized agent.
  2. Identification of the copyrighted work claimed to be infringed.
  3. Identification of the material claimed to be infringing, with information sufficient to locate it (typically the URL of the page or post).
  4. Your contact information (name, address, phone, email).
  5. A statement that you have a good-faith belief the use is not authorized by the copyright owner, its agent, or the law.
  6. A statement under penalty of perjury that the information in the notice is accurate and that you are the owner or authorized to act on the owner's behalf.

Counter-notice

If your content was removed and you believe it was a mistake or misidentification, you may submit a counter-notice with the same level of detail above. We'll forward it to the original complainant; if they don't file suit within 10–14 business days the content may be restored.

Repeat infringers will have their accounts terminated under our standard Terms of Service.
